The crash involved a 2007 Lexus SUV operated by Jason Henry of Wooster and a 2021 Peterbilt tractor-trailer operated by a 30-year-old male driver.

Preliminary investigation indicates that Henry was traveling west on Jeffrey Road and failed to yield for cross-moving traffic at the stop sign at Lafayette Road.

His car was struck by the northbound tractor-trailer.

Investigators indicated that Henry was not wearing a seat belt and died at the scene. They said speeding was likely a contributing factor in the crash.

Two juvenile passengers in the vehicle were transported by medical helicopter to area hospitals for treatment. The driver of the tractor-trailer, who was the sole occupant of the truck, was treated at the scene and later released.

The crash remains under investigation by the Medina County Sheriff’s Office and the Medina County Coroner’s Office. Agencies assisting at the scene included the Ohio State Highway Patrol Medina Post, the Lodi Police Department, and the Town and Country Fire District.
