“We are continuing our efforts to locate 48-year-old Chad Polen, who was last seen on June 19th, 2025, at approximately 6:22 a.m., walking away from his residence,” the department posted on Facebook. “As of this time, both Chad and the vehicle remain unlocated.”

Polen is a disabled veteran, according to the Always Watch And Recognize Your Environment (AWARE) foundation, and requires medication. He was last seen walking away from his residence at Perry Hills Colony Apartments.

“According to his family, he rarely leaves the apartment and has never been gone this long,” the foundation said in a Facebook post. “He is possibly driving a blue 2016 Honda Accord with OH license plate EVJ2737.”

Anyone with information on Polen’s whereabouts can contact the Perry Township Police Department at 330-478-5121.
