The Peoria City/County Health Department conducts food inspections for a wide range of establishments in the area. Inspections look for issues that could contribute to the spread of foodborne illness, as well as general cleanliness and maintenance of the establishment.
The Journal Star examined routine inspections for restaurants, mobile establishments and bars in Peoria County. There were 87 inspections for these categories in June.
The establishments listed below received either no violations or enough violations to warrant a warning from the health department. A section on establishments with 10 or more violations total − with no warning comment − is also included.
According to the Peoria City/County Health Department, “inspections only provide a snapshot of a food establishment’s practices, as they are based on what is seen at the time of the inspection.”

Bake and Brew Panaderia at 5201 W. War Memorial Drive, Suite 530, Peoria, had two priority foundation violations on June 26.
However, a comment noted “an ongoing violation with labels on prepackaged products” could lead to a compliance conference if not corrected. The inspector found pre-packaged goods had not been labeled, and foods packaged at the bakery were missing labels indicating “each major food allergen contained in the food.”
Lakemart at 10013 W. Lancaster Road, Mapleton, received three priority, five priority foundation and eight core violations and had its license suspended on June 12 due to “the amount of Priority and Priority Foundation Violations, as well as not having a food safe sanitizer on site.”

Osaka Japanese Restaurant at 5201 W. War Memorial Drive, Suite 502, Peoria, received three priority, one priority foundation and 13 core violations. This included 10 repeat violations.

Sonic Drive Inn #5962 at 4305 N. Sterling Ave., Peoria, received one priority, one priority foundation and nine core violations.
The routine inspection, conducted on June 16, was done alongside a complaint investigation and third follow-up inspection for allergen awareness certificates. The complaint noted: “the store looks dirty through drive thru window and looks to be not cleaned at the prep table. There is feces in the men’s urinal, urine all over the floor, the toilet has feces on the seat and running down the side.”

Comments on the follow-up inspection noted the restaurant would “be referred to the Director of Environmental Health for a compliance conference to determine additional enforcement action due to continued noncompliance with the Illinois Food Code and Chapter 10 Food Safety, of the Peoria County Code.”
Steak N Shake #547 at 7515 N. Grand Prairie Drive, Peoria, received one priority and six core violations on June 1.
Comments, however, noted an ongoing violation of the restaurant not having a Certified Food Protection Manager, noting: “Should the establishment not be under the control of a certified food (protection) manager in 30 days, the establishment will be called for a compliance conference with the Director of Environmental Health.”
Warning comment follow-up inspections
Lakemart at 10013 W. Lancaster Road, Mapleton, received three priority, five priority foundation and eight core violations and had its license suspended on June 12.
The license was reinstated on June 15 after a follow-up inspection found priority and priority foundation violations had been corrected.
A 10-day follow up on June 25 found one priority, two priority foundation and two core violations, including the inspector finding a “commercially sealed bag of powdered French vanilla to have pest bite/chew marks on corner of bag, with droppings around the bag.”
Steak N Shake #547 at 7515 N. Grand Prairie Drive, received one priority and six core violations on June 1. The routine inspection noted the potential for a compliance conference, however, if the restaurant was not “under the control of a certified food (protection) manager in 30 days.”
The health department noted the violation had been corrected on June 12.
