Springfield-area nursing homes fined for patient safety violations

SPRINGFIELD – Four times a year, an Illinois agency releases a report showing violations against nursing homes, and how much the facilities were fined.

The Illinois Department of Public Health recently released its first quarter report that spans from January to March of 2026, though many of the surveys happened prior to the first quarter.

Here are facilities within about 45 minutes of Springfield that were fined for violations.

Arc at Sangamon Valley, Springfield

Address: 3400 W. Washington St.

Fined: $2,200

Survey date: Oct. 16, 2025

What: Health officials found the facility failed to safely transfer a resident to prevent falls. The failure resulted in a resident falling and receiving a laceration.

The Grove Health & Rehab Center, Jacksonville

Address: 873 Grove St.

Fined: $25,000

Survey date: Nov. 20, 2025

What: A resident at the facility hurt their head after falling out of bed. Officials found the facility failed to provide a safe transfer for the resident ahead of the fall.

Imboden Creek Senior Living, Decatur

Address: 180 W. Imboden Drive

Fined: $25,000

Survey date: Nov. 6, 2025

What: A resident died at the facility on Oct. 16, 2025, after officials said the facility failed to provide emergency response resuscitative efforts in a timely manner, resulting in a 10-minute delay in providing CPR.

Loft Rehab of Decatur

Address: 500 W. McKinley Ave.

Fined: $25,000

Survey date: Oct. 9, 2025

What: The state found a resident in a wheelchair suffered fractures to the humerus, fibula and tibia after not being properly secured in the wheelchair while being transported in a van. The resident became immobile after the injuries, according to officials.

Fair Havens Senior Living, Decatur

Address: 1790 S. Fairview Ave.

Fined: $29,400

Survey date: Dec. 10, 2025

What: The facility was fined for multiple reasons, including protecting the dignity and psychosocial wellbeing for two residents, with feelings of disrespect. The facility also failed to properly transfer a resident, resulting in bruising and pain to the resident’s leg. Lastly, a resident was hospitalized after developing a UTI after the facility failed to provide hygienic catheter care and monitoring urinary catheter output and timely treating UTI symptoms.

St. Clara’s Rehab & Senior Care, Lincoln

Address: 1450 Castle Manor Drive

Fined: $27,200

Survey date: Dec. 17, 2025

What: A resident was hospitalized and needed surgery after the facility failed to implement pressure relieving interventions to prevent a facility acquired pressure ulcer. Another resident was taken to the hospital after they fell in the bathroom and hit their head. Officials said the facility failed to ensure safety the resident, who was diagnosed with dementia and muscle weakness.

Address: 565 W. Marion Ave.

Fined: $4,400

Survey date: Oct. 14, 2025

What: Officials said the facility failed to ensure adequate supervision was provided and two residents fell, resulting in a broken collarbone for one resident and swelling of blotted blood on another resident’s forehead. The facility also failed to identify and report an alteration in skin integrity resulting in a pressure ulcer in another resident.

Address: 565 W. Marion Ave.

Fined: $2,200

Survey date: Jan. 15, 2026

What: The facility failed to satisfy general requirements for nursing and personal care. This is evidenced by one resident’s medical record documents medical diagnoses as encephalopathy, falls, vascular dementia with behavioral disturbance, abnormal findings on diagnostic imaging of other parts of musculoskeletal system, anemia, weakness and pacemaker.
