Eight Springfield-area nursing homes fined thousands of dollars

SPRINGFIELD – Four times a year, an Illinois agency releases a report showing violations against nursing homes and the associated fines.

The Illinois Department of Public Health has released its third quarter report that spans from July to September in 2025.

Here are facilities within about 45 minutes of Springfield that were fined for violations.

Arc at Sangamon Valley, Springfield

Address: 3400 W. Washington St.

Fined: $25,000

Survey date: March 5, 2025

Based on interview and record review, the facility failed to assess and identify impaired skin integrity, failed to document weekly skin assessments, and failed to follow physician orders for pressure ulcer treatment for three residents.

The failure resulted in one resident developing a pressure ulcer that was classified as an unstageable/stage four, requiring significant debridement on multiple occasions, osteomyelitis and a seven-day hospital stay.

Survey date: March 15, 2025

Based on interview, observation, and record review, the facility failed to monitor a resident’s enteral nutrition needs, monitor a resident’s weight, identify severe weight loss of a resident, provide needed interventions to prevent further weight loss, and re-assess a resident’s nutritional needs when the resident was not tolerating enteral nutrition.

The failure resulted in one resident experiencing a 11.98% weight loss in six weeks of being admitted to the facility.

Villa Health Care East, Sherman

Address: 100 Marian Parkway, P.O. Box 109

Fined: $25,000

Survey date: July 17, 2025

Based on record review and interview, the facility failed to ensure adequate supervision and precautions in place for one resident.

The failure resulted in the resident falling off the bed, fracturing her femur, and requiring surgical repair.

Lincoln Village Healthcare, Lincoln

Address: 2202 N. Kickapoo St.

Fined: $2,200

Survey date: March 15, 2025

Based on interview, observation and record review, the facility failed to provide supervision and implement fall prevention interventions to prevent resident falls for two residents.

The failures resulted in one resident falling and sustaining an injury to his or her right eyelid, and the other resident falling and experiencing left hip pain.

Prairie Village Healthcare Ctr, Jacksonville

Address: 1024 W. Walnut St.

Fined: $2,200

Survey date: March 15, 2025

Based on interview, observation, and record review, the facility failed to identify and prevent a pressure ulcer for one resident.

The failure resulted in the resident developing a pressure ulcer on the left heel while at the facility.

Sunrise Skilled Nursing Rehab, Virden

Address: 333 S. Wrightsman St.

Fined: $25,000

Survey date: March 22, 2025

Based on interview and record review, the facility failed to obtain physician-ordered laboratory testing in one resident.

The failure resulted in the resident being admitted to the hospital. The resident was diagnosed in the hospital with Supratherapeutic INR (Initial Normalized Ratio) with a level greater than 10 (target range is between 2-3) and had to receive medication to reverse the effects from the anticoagulant, Warfarin, that the resident was receiving in the facility for a diagnosis of Pulmonary Embolism.

Imboden Creek Senior Living, Decatur

Address: 180 W. Imboden Drive

Fined: $2,200

Survey date: July 3, 2025

Based on interview and record review, the facility failed to conduct a fall investigation and failed to promptly notify a physician of a resident’s change in condition and significant decline in mobility, which resulted in prolonged discomfort/pain for one cognitively impaired resident.

More than 24 hours after the initial change of condition was noted, an X-ray was obtained and the resident was sent to the Emergency Room, admitted, and had surgical repair of a left intertrochanteric hip fracture.

Loft Rehab of Decatur, Decatur

Address: 500 W. McKinley Ave.

Fined: $25,000

Survey date: March 1, 2025

Based on interview and record review, the facility failed to immediately notify the physician, complete a pain assessment, complete a physical assessment, and provide pain management when severe pain with redness and swelling to the left knee began suddenly and continued for one resident.

The failures resulted in the resident experiencing severe pain from March 9 to March 14, 2025, when he or she was hospitalized with a left femur fracture, which required surgical repair.

Arc at Hickory Point, Forsyth

Address: 565 W. Marion Ave.

Fined: $4,400

Survey date: April 17, 2025

Based on observation, interview, and record review, the facility failed to identify, assess, intervene, and treat pressure wounds for three residents.

The failures resulted in two residents developing facility-acquired unstageable wounds underneath immobilizers.

Arc at Hickory Point, Forsyth

Address: 565 W. Marion Ave.

Fined: $2,200

Survey date: March 8, 2025

Based on observation, interview, and record review, the facility failed to identify one resident’s Sacral Deep Tissue Injury, failed to obtain and provide treatment orders, failed to update a care plan timely, and failed to implement pressure reducing interventions for three residents.

As a result of the failures, one resident had pain from her Stage three Sacral pressure ulcer, which was acquired and worsened under the care of the facility.
