A preliminary investigation found a 16-year-old Campbellsport resident was driving an eastbound SUV on Haven Drive, stopped at a stop sign, then failed to yield while attempting to cross County Highway V, the sheriff’s office said.

The SUV entered the path of a northbound SUV driven by the 55-year-old Campbellsport woman, authorities said.

The woman, who was the only person in the northbound SUV, was taken by ambulance to SSM Health in Fond du Lac with serious injuries.

The 16-year-old driver and two juvenile passengers in the eastbound SUV were taken by ambulance to Froedtert Hospital in West Bend, where they were treated for minor injuries.

The crash remains under investigation by the Fond du Lac County Sheriff’s Office. Campbellsport Fire Department, Campbellsport Ambulance and the Campbellsport Police Department assisted at the scene.
