'Explosive diarrhea' illness linked to food parasite is rising in NY

Health officials are tracking a rise in cyclosporiasis cases in New York and across the United States as investigators work to identify possible sources of exposure linked to the foodborne parasite.

As of July 10, preliminary data from New York state health officials showed about 511 cases of cyclosporiasis statewide since May 1, among the highest totals in the country.

New York typically reports about 500 to 700 cyclosporiasis cases each year, with similar totals reported in 2024 and 2025, according to state health officials.

Nationally, the Centers for Disease Control and Prevention said it has confirmed 843 domestically acquired cases in 31 states this year and is reviewing more than 1,500 additional reports. Michigan has reported more than 1,500 infections, making it the hardest-hit state.

Health officials said there is currently no evidence that all cases are connected to a single nationwide outbreak. Investigations into potential sources of exposure are ongoing.

What is cyclosporiasis?

Cyclosporiasis is an intestinal illness caused by the microscopic parasite Cyclospora cayetanensis.

People become infected after consuming food or water contaminated with the parasite. In the United States, outbreaks have often been linked to fresh produce, including cilantro, basil, salad mixes, snow peas and raspberries, according to health officials.

Unlike many stomach illnesses, cyclosporiasis is not typically spread directly from person to person.

Symptoms typically begin about a week after exposure but can take up to two weeks to appear. Without treatment, illness can last for weeks or longer.

Cyclosporiasis cases rising in New York

New York health officials said cyclosporiasis cases typically increase during the spring and summer months.

The recent increase has prompted state and local health departments to work with the CDC and the U.S. Food and Drug Administration to identify possible sources of contamination.

Most New York cases have been reported in New York City, though infections have also been identified in other parts of the state. Officials said the current increase does not appear to be linked to a single source.

Cyclosporiasis is usually not life-threatening, but severe diarrhea can lead to dehydration, especially among older adults, young children and people with weakened immune systems.

People experiencing persistent diarrhea, signs of dehydration, severe abdominal pain or symptoms lasting more than several days should contact a health care provider.
