Indiana hospital safety report released. Here's how Monroe and IU Health Bloomington rate.

Monroe County’s two hospitals — Monroe Hospital and IU Health Bloomington Hospital — were included in the fall 2025 hospital safety and quality report by nonprofit The LeapFrog Group.

Monroe Hospital received B overall while IU Health Bloomington Hospital received a C. The study included 73 hospitals in Indiana and graded each on five areas: infections, problems with surgery, safety problems, practices to prevent and doctors, nurses and hospital staff. Both hospitals did not provide information for all of the categories. IU Health Bloomington Hospital received the worse hospital scores for several areas.

The LeapFrog Group is nonprofit watchdog organization founded in 2000. It collects, analyzes and disseminates data about hospitals and health care choices in the U.S. Each spring and fall, LeapFrog releases a report that grades hundreds of hospitals.

In Indiana for the fall 2025 report, there were 15 hospitals that received an A, 24 that received a B, 23 that received a C, 10 that received a D and one that was not graded.

What the report says about Monroe Hospital

For infections, Monroe Hospital did not provide information about MRSA, infection in the blood, urinary tract and surgical site infections. For C diff infections it was given a “worse than average” score; for sepsis infection after surgery it was rated as average.

For problems with surgery, Monroe Hospital received the highest “better than average” score for dangerous objects left in patient’s body. It received average scores for blood leakage, kidney injury after surgery, serious breathing problems and accidental cuts and tears. Not available were surgical wound splits open and death from serious treatable complications.

For safety problems, Monroe Hospital was better than average for harmful events, patient falls and injuries and air or gas bubbles in the blood. It had an average score for dangerous bed sores, falls causing broken hips, collapsed lung and dangerous blood clots.

For practices to prevent, Monroe Hospital had better than average scores for doctors order medication through a computer, safe medication administration, handwashing and staff work together to prevent errors. It was average for communication about discharge and was worse than average for communication about medicines.

For doctors, nurses and hospital staff, Monroe Hospital was better than average for effective leadership to prevent errors, nursing and bedside care for patients, communications with doctors, communications with nurses and responsiveness of hospital staff. It was worse than average for specially trained doctors care for ICU patients.

What the report says about IU Health Bloomington Hospital

For infections, IU Health Bloomington Hospital received better than average for infection in the urinary tract. It had an average score for C. diff infection, infection in the blood and sepsis infection after surgery. The hospital had worse than average ratings for MRSA infection and surgical site infection after colon surgery.

For problems with surgery, IU Health Bloomington Hospital was better than average for dangerous object left in patients body and serious breathing problems. It was average for accidental cuts and tears and death from serious treatable complications. It was worse than average for surgical wound splits open, blood leakage and kidney injury after surgery.

For safety problems, IU Health Bloomington Hospital was better than average for harmful events, dangerous bed sores and air or gas bubble in the blood. It was average for patient falls and injuries, dangerous blood clot and falls causing broken hips. It was worse than average for collapsed lung.
