73 Indiana hospitals have been graded for patient safety by The Leapfrog Group. Here's how they scored

The Leapfrog Group released The Leapfrog Hospital Safety Grades for fall 2025. Each hospital’s grade is solely based on patient safety in each hospital and takes into consideration a hospital’s overall performance in keeping patients safe from preventable harm and medical errors.

The organization grades hospitals twice a year. 73 Indiana hospitals were graded for fall 2025.

What is Leapfrog Hospital Safety Grade?

The Leapfrog Hospital Safety Grade is a public service provided by The Leapfrog Group, an independent nonprofit organization committed to driving quality, safety and transparency in the U.S. health system.

How are hospitals graded for The Leapfrog Safety Grade?

According to their website, The Leapfrog Hospital Safety Grade uses up to 32 evidence-based measures of patient safety.

Taken together, those performance measures produce a single letter grade representing a hospital’s overall performance in keeping patients safe from preventable harm and medical errors.

The Leapfrog Hospital Safety Grades are issued twice a year, once in the spring and once in the fall. The grade of each hospital can fluctuate from its previous grade.
