A report released by Wichita Falls police Monday has revealed details of an accident that claimed the life of a 70-year-old woman in early 2026.
The report was released in response to a second open records request submitted to the city by the Times Record News.
Mary Alice Perez was killed as she attempted on foot to pick up a grandchild at Crocket Elementary School at 1900 Arthur Street about 1:50 p.m. on Jan. 13, the police report said.
Vehicles were lined up to pick up children, the police report said. Perez walked across the street and stepped between two vehicles as they started to move forward.
Faye Canafax, who was driving a 2026 Hyundai Santa Fe, told an officer she had applied her brakes to stop as Perez walked between her vehicle and a gray Santa Fe.
Canafax said her foot slipped off the brake and hit the accelerator, causing her vehicle to pin Perez between the two vehicles briefly.
The police report said Perez was pinned until hydraulic lifting equipment could free her.
Also as a result, Canafax’s vehicle caused a collision between the gray Santa Fe driven by Ann White and a Honda Odyssey driven by Sarah Garrett, the police report said.
No one else was injured in the incident, according to the police document.
Perez was taken to United Regional Health Care where she was pronounced dead from her injuries at 4:30 p.m., police said.
The police report concluded, “No criminal charges are being filed, as the investigation found no evidence of gross negligence or recklessness.”
Initial documents released by the city on Feb. 19 in response to a TRN open records request didn’t include key details about the accident. TRN filed a second open records request to obtain details that are typically routinely provided by law enforcement agencies.
