Clay Township, MI – Community First Health Centers will open a new school-based health center at Algonac Jr./Sr. High School on March 17, 2026, expanding access to medical and behavioral health services for children and adolescents in the area, according to a Community First press release issued March 6.
The clinic, located inside the school at 5200 Taft Road in Clay Township, will serve individuals ages 5 through 21. Residency in Algonac or Clay Township is not required to receive care.

The new clinic follows a 2025 decision by the St. Clair County Health Department to consolidate primary care services at its Main and Teen clinics in Port Huron, ending services at sites in Yale, Capac and Algonac.
While Yale and Capac have not reopened, the Algonac location is returning through a new partnership with Community First Health Centers.
“I am very excited that Community First Health has agreed to step in and provide these valuable services for students and families in Algonac, and we look forward to a great partnership in the years to come,” said Al Latosz, superintendent of Algonac Community Schools.
According to Community First Health Centers, the clinic will offer preventive care, sports physicals, illness treatment, chronic condition management and on-site testing.
Youth and adolescent counseling services will also be available for students experiencing depression, anxiety, crisis situations or challenges at home or school.
Medical care will be provided by Haleigh Groomes, a board-certified family nurse practitioner and graduate of Oakland University. Groomes has eight years of nursing experience and previously worked in urgent care and public health before joining Community First Health Centers in December 2025.
Community First Health Centers is also recruiting a licensed Master of Social Work to provide additional behavioral health services at the site.
The clinic will be open Tuesday through Thursday from 7:30 a.m. to 4 p.m., with after-hours services available for established patients. Appointments can be scheduled by calling 810-777-9924.
Community First Health Centers is a federally qualified health center that provides medical, dental and behavioral health services across southeast Michigan regardless of insurance status or ability to pay.
