Regency Manor, an assisted living facility in Independence, Kentucky, is once again under state investigation after officials received multiple complaints, said Beth Fisher, deputy executive director of the Kentucky Cabinet for Health and Family Services.
Following the complaints, the Cabinet for Health and Family Services investigated Regency Manor on June 18 and cited health and safety concerns. The Office of Inspector General is still investigating.
The investigation comes at a time when Regency Manor is already in the public eye. A volunteer at the facility, Rachel Paxitzis, was recently fired for posting on social media asking for help fixing up the place. Managers asked her to leave the property after Paxitzis’ Facebook posts brought attention to Regency Manor’s condition and recruited more volunteers, who later filed complaints to the state.
The personal care center has long been subject to legal questions. In 2025, the Cabinet for Health and Family Services attempted to shut down Regency Manor for putting residents in danger. The lawsuit was later dismissed.
Cited in the Kenton County Circuit Court lawsuit were violations including medication errors, temperature control problems, medical record issues and failure to keep a “clean, safe, sanitary, order free environment.”
“A common reaction to these serious violations is one of dismissal, condescension, and avoidance,” the lawsuit states.
Two citations listed in the suit also detail instances where Regency Manor failed to ensure the safety of its residents. In 2024, a resident diagnosed with schizophrenia, Parkinson’s disease and vision problems who had a history of leaving the facility wandered away from Regency Manor and was injured after being struck by a car.
Only two years earlier, another resident of Regency Manor − Sherry Moore − was missing for 2.5 hours before staff reported her missing. Police found Moore’s body roughly 300 yards behind the facility six days later. The case is ongoing.
The owners of Regency Manor, Eldercare Partners, could not be immediately reached for comment, but a manager at the facility told The Enquirer on Monday that no one there could respond.
