Ann Arbor restaurant inspections find moldy ingredients, rusty cans

Recent health inspections in some Ann Arbor restaurants revealed rusty cans, moldy onions and more between February and April, according to Washtenaw County Health Department inspection reports.

Many of the violations have been corrected. The presence of violations in a past inspection does not necessarily mean the establishment has the same violations today.

Below is a list of violations found at a few restaurants in Ann Arbor.

Levana, 3148 Packard St.

During a routine inspection on February 10, an inspector noted several problems at Levana.

∎Observed the following in the walk-in cooler that is not working:

1. 2 cases of raw beef 52F.

 2. 4 cases of raw chicken 52F.

3. 6 house made dairy based sauces 52F. Corrected by PIC discarding all noted items into the dumpster.

 ∎Observed 3 cans of ketchup, 2 cans of beets and 2 cans of pickles in open metal cans with rust present inside the can, located in the walk-in cooler, large double door cooler outside of the walk in cooler and the front prep table cooler. Corrected by PIC discarding all 7 noted cans.

 ∎Observed a case of raw eggs stored above ready to eat food in the large double door cooler nearest to the walk-in cooler. Corrected by PIC moving the raw eggs to the bottom shelf.

∎Observed one case of moldy onions, located in the walk-in cooler. Corrected by PIC discarding the noted items into the dumpster.

∎Observed the hand sink to have been removed on the cookline. Note: There was no other hand sinks near or in view from the cookline. To correct reinstall the hand sink, hand wash sign, paper towel and soap.

∎Observed the walk-in cooler (WIC) unable to keep cold food at 41F or below. To correct discontinue use of the WIC until it can be repaired to hold cold food at 41F or below.

Bewon, 3574 Plymouth Road

 During a routine inspection on March 6, an inspector noted several problems at Bewon.

∎Found raw meats, containers of chicken, beef and fish stored on shelf above and next to ready to eat food in the 3 door reach in cooler. Found raw shell eggs and beef stored on shelf above and next to ready to eat food in the 2 door reach in cooler. PIC corrected by relocating raw meats to bottom shelves of coolers below and away from ready to eat food.

∎Found no consumer advisory on electronic menu where egg on bibimbap can be order undercooked. To correct provide consumer advisory on menu.  2 consumer advisory options: Option 1: Put the following statement on the main menu: “Ask your server about menu items that are cooked to order or served raw. Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase you risk of foodborne illness.” Option 2: Identify undercooked animal-based foods with a disclosure and reminder. Example: Menu item: *Fried egg on the menu: *Contains raw or undercooked ingredients. Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase you risk of foodborne illness.

∎Found no hot water flowing from faucet in the unisex restroom. PIC states small hot water tank supplying only the restroom faucet has failed. To correct provide hot water in handsink.

∎Found ready to eat (RTE) TCS food, a large container of pasta and facility made sauce without 7 day (day prepared + 6 days) datemark in the 3 door prep top cooler. PIC corrected by providing datemarks for noted items.

∎ Found the tiled floor under and around the dishwashing 3 compartment sink broken and chipped. To correct repair tiled floor.

∎Found cases of single use utensils stored on the floor by the front counter area. To correct store utensils on shelf at least 6 inches off the floor.

∎Found opened bag of sugar and MSG not covered in the storage area. To correct store food items in a protected manner, such as covering sugar and MSG.

∎Found several wood spoons on a container in the cooking and prep area. To correct replace wood spoons with commercial grade utensils, such as stainless steel spoons.

∎ Found a gas rice cooker on the counter and not under a cooking hood. To correct either remove gas rice cooker or provide/place under cooking hood.

∎ Found the ceiling lights in the kitchen not fully functioning. To correct repair/replace lights.

A Plus BBQ, 1759 Plymouth Road

 During a routine inspection on March 20, an inspector noted several problems at A Plus BBQ.

 ∎Found raw seafood stored next to ready to eat (RTE) food, lettuce on the top portion of the 2 door prep top cooler on the cooking line. Found raw meat stored above RTE food in the bottom portion of the 2 door prep top cooler on the cooking line. Found raw meat stored above container of RTE food, kimchi in the walk in cooler. PIC corrected by relocating raw meats below and away from RTE food in the 2 door prep top cooler and walk in cooler. Management has been instructed to monitor and ensure raw meats are stored below and away from RTE food in coolers.

∎ Found the underside of the slicer blade and surfaces surrounding blade on slicer soiled with debris. Found food debris on the cutting blades of cutting machine. To correct follow manufacturer’s directions and clean slicer and cutting machine. PIC corrected by having employees wash, rinse and sanitize slicer and cutting machine.

∎Found rice warmers on wood chairs in the kitchen. To correct store rice warmers on a commercial grade prep table.

∎Found ice machine in the back kitchen area not working. To correct repair ice machine.

∎ Found broken cutting machine on the floor in the back kitchen area. To correct removing cutting machine from facility if no longer needed for operations.

∎Found current food license not posted in front door area. To correct post current food license.

∎Found no antichoking poster on kitchen wall. To correct post antichoking poster on kitchen wall.

 BaoBao Asian Eatery, 3365 Washtenaw Ave.

During a routine inspection on February 11, an inspector noted several problems at BaoBao Asian Eatery.

∎Observed the following items that the PIC said were cooked the night before and did not meet the cooling time frames as they were still in the process of cooling. Items were located in the walk in cooler.

1. Beef stock 45F.

2. Cubed pork 47F. Corrected by PIC discarding all the noted items into the garbage.

∎Observed the following in the walk in cooler:

1. 2 cases of raw eggs stored above ready to eat food.

2. Raw chicken stored with and above raw beef, raw seafood, raw pork and ready to eat food.

 3. Raw whole beef stored with and above ready to eat food.

4. Raw seafood stored with and above ready to eat food.

5. Raw pork stored with above ready to eat food.

Corrected by PIC reorganizing the walk-in cooler, in the following way: 1. Raw chicken was alone on the bottom shelf. 2. Raw eggs, raw pork, raw seafood and raw whole beef were stored on a separate shelf above raw chicken and below ready to eat food.

∎Observed the following items that the PIC said were cooked the night before, covered and still in the cooling process. Items were located in the walk in cooler. Note: There was condensation on the covering. 1. Beef stock 45F. 2. Cubed pork 47F. Corrected by PIC discarding all the noted items into the garbage.

∎Observed 6 cans with large dents along the seams of the can, located in dry storage. Corrected by PIC putting the cans in a newly created area for dented cans to be returned.

∎ Observed the following frozen items, out on the counter thawing, located in the prep area: 1. Case of pork. 2. Case of beef. 3. Case of fish. Corrected by putting the noted items into the walk-in cooler to thaw.

∎ Observed 8 open metal cans of ketchup, bamboo, pineapple and hoison sauce. Items were located in the top loading cooler on the cookline and in the walk-in cooler. Corrected by PIC transferring the noted food into food grade reusable containers.

∎Observed boxes with food inside stored on the floor of the dry storage room. To correct reorganize the dry storage room so that items are not stored on the floor.

Hungry Howies Pizza, 2026 W. Stadium Blvd.

During a routine inspection on February 10, an inspector noted several problems at Hungry Howies Pizza.

∎Observed bare hand contact with ready to eat salad toppings. Salad prep cooler has been removed and salad and pizza toppings are now in the same prep cooler. Observed employees touching pizza items like cheese, tomatoes, olives, etc. when making pizza. There are the same items used on salad. Ready to eat food cannot be touched with bare hands. To correct, wear gloves for pizza and salad items or install a separate salad prep cooler and keep salad items in that cooler. Tongs, gloves, or deli tissue must be used when handling/touching ready-to-eat food.

∎Observed no certified food manager. Establishment must have at least 1 certified food manager. Must work at least 30 hours a week in the establishment. Can not use certificate at more than one location. To correct, provide a certified food manager certificate or proof of enrollment within 10 days.

∎Observed a unused a two door and a one door prep cooler stored near the dish area. Unused equipment must be removed from the facility. To correct, remove or use that prep coolers.

∎Observed water damage on a ceiling tile and a ceiling tile not in place. Manager states that ceiling leaks when it rains. Observed wet spot on floor. Physical facilites must be in good repair. To correct, fix leak and reinstall ceiling tile.

Leo’s Coney Island, 160 S. Zeeb Road

 During a routine inspection on March 16, an inspector noted several problems at Leo’s Coney Island.

∎Observed lemon rice soup at 71-124 degrees reheating in the steamer unit at the front counter. Potentially hazardous food must be reheated rapidly to at least 165 degrees using microwave, stove, oven, etc. Steam unit can not be used to reheat food. To correct discard lemon rice soup or reheat to 165 degrees to hold above 135 degrees. Corrected at time of inspection by education and by the cooks reheating the soup above 165 degrees and holding above 135 degrees.

∎Observed two cream pie slicers, a container of ham bits, a 5 gallon bucket of lemon rice soup and tuna outdated in the coolers. Potentially hazardous food can not be used after the use by date. To correct, discard the outdated food and monitor food dates. Corrected at time of inspection by education and by the manager discarding the outdated food.

∎Observed the excess food debris on the following food contact surfaces: -Both dicer’s on the storage shelf -Slicer on and near the blade. Food contact surfaces must be clean to the sight and touch. To correct, clean and sanitize these surfaces and maintain clean.

∎Observed improper cooling practices. Observed three 5 gallon buckets of lemon rice soup cooling on the prep table in the prep area. Manager states that if was cooked about an hour ago. Manager states then place the buckets of soup in the walk in cooler to final cool. Potentially hazardous food must be cooled rapidly using shallow pans, ice baths. Ice paddles or a combination of methods. To correct, use shallow pans, ice baths, ice paddles or other methods to cool soup rapidly. Manager places the soup buckets in the walk in freezer to cool and time of inspection. Manager states they will stir and monitor the temperature of the soup while cooling.

∎Observed low water pressure at the left side hand washing sink in the womans restroom. Water just trickles out. Plumbing system must be maintained in good working order. To correct, provide more hot and cold water pressure to the left side hand washing sink in the womans restroom.

∎ Observed two large tires in the establishment near the mop sink. Items that do not pertain to the food establishment can not be in the establishment. To correct, remove the tires from the premises.

∎Observed two white cutting boards blackened with deep groves. Cutting boards can not have deep grooves. To correct, discard damaged cutting boards and use cutting boards that do not have deep grooves.

∎Observed no lid or sanitary napkin bin in the employee restroom. Restroom used by females must have a sanitary napkin bin or a lid on the garbage can. To correct, provide a sanitary napkin bin or a lid for the garbage can in the employee bathroom.

∎Observed excess dust on the ceiling tiles around the vents on the cook line. Physical facilities must be clean. To correct, clean the ceiling and vents on the cook line and maintain clean. Taste of India, 217 S. State St. During a routine inspection on February 9, an inspector noted several problems at Taste of India.

∎Observed food cooled improperly . Observed two 10 gallon containers of tomato gravy at 58=68 degrees cooling in the walk in cooler. Manager states they were made yesterday. Manager discarded both containers of tomato gravy at time of inspection. Potentially hazardous food must be cooled from 135 degrees to 70 degrees in 4 hours or less and from 70 degrees to 41 degrees in 4 hours or less. To correct, cool potentially hazardous food rapidly in proper time frame.

∎Observed a cook line employee touching a cellphone and then touching clean utensils and pans without washing hands. Employees must wash hands when soiled and after touching cell phones. To correct, train employees on when to wash hands. Corrected at time of inspection by education and by employees washing hands at correct times.

∎Observed improper cooling practices. Observed two 10 gallon containers of tomato gravy cooling in the walk-in cooler. Potentially hazardous food must be cooled rapidly. To correct, use ice paddles, shallow containers. Ice baths or a combination of methods to cool potentially hazardous food rapidly.

∎Observed hand washing sink in the in the dish area covered with a strainer inside and a tray on top of it. Hand washing sinks must be fully accessible at all times. To correct, remove items from on top and inside hand washing sink.. Corrected at time of inspection by education and by the manager removing the items from the hand-washing sink in the dish area.

∎Observed two employees preparing food without a hat or hair net. All employees preparing food must wear an effective hair restraint like a hat or hair net when preparing food. To correct, have employees wear a hat or hair net when preparing food. Corrected at time of inspection by education and by employees putting on a hat.

∎Observed plastic and paper cups used to dispense soups, in sugar and 8 other dry storage food containers. Cups were stored in the food product. Scoops with handles must be used. To correct, use a scoop with handle and store inside the food container with the handle up or outside of the container on a clean surface.

∎Observed the following equipment in need of repair: -Torn door seals on the 2 door cook line prep cooler equipment must be maintained in good working order. To correct, provide new door seals for the 2 door cook line prep cooler.

∎Observed excess food debris on the following non food contact surface: -Top shelf above the cook line prep cooler and wells non food contact surfaces must be clean. To correct, clean this non food contact surface and maintain clean.

∎Observed excess debris on top of the dish machine. Dish machines must be clean. To correct, clean the top of the dish machine.

∎Observed the following plumbing issues: Plumbing system must be maintained in good working order -Observed low cold water pressure at the hand washing sink near the ice machine. To correct, provide more water pressure.

∎Observed towels used as drain plugs at the 3 compartment sink. To correct, provide drain plugs/stoppers for the sink.

∎ Observed 4 light fixtures in the kitchen not working. Adequate lighting must be provided. To correct, fix the lights in the kitchen to help facilitate cleaning and prevent accidents.

∎Observed no lid on the garbage can in the employee restroom or woman’s restroom. Woman’s and employee restrooms must have a sanitary napkin bin or a lid on the garbage can. To correct, provide a lid on the garbage can or a sanitary napkin bin in the woman’s and employee restroom.
