Established in 2006, Grace Hospice is a Community Health Accreditation Partner (CHAP) accredited hospice agency providing patient and family-centered care that optimizes the quality of life. Our care team works together focusing on the patient’s needs in their home, home of a loved one, or in a long term care or assisted living facility.
Grace Hospice is a member of the National Hospice and Palliative Care Organization (NHPCO). In addition, we are national partners of We Honor Veterans, a pioneering campaign developed by National Hospice and Palliative Care Organization in collaboration with the Department of Veterans Affairs. Our Marysville Branch has successfully completed level 4 of the We Honor Veterans program and currently pursuing Level 5 status.

For Patients & Families
Hospice is a philosophy of caring for patients whose medical conditions are not responding to treatment or for those patients who choose not to seek aggressive medical care at the end stage of life. It is a program for patients who are considered terminal and are expected to live six months or less if the disease follows its natural course, although many patients outlive this time frame.
The hospice philosophy focuses on comfort care not curative care. The goal of hospice care is for the patient to remain as comfortable and pain-free as possible in their own home or the home of a loved one through the end of life.
Hospice care is provided in any home or facility setting (independent residence, assisted living, adult foster care, and group homes) that the patient considers home.

Although all hospices are governed by the same rules and regulations, also called “Conditions of Participation,” not all hospices are the same. Grace Hospice offers the unique advantage of a continued close relationship with your primary care physician, and also the ability to connect you and your loved one with a physician who offers house calls. Quality of life starts with excellent care provided by our local team of physicians, nurses, hospice aides, social workers, chaplains and volunteers. All of our locations are CHAP accredited – one of the most stringent accrediting bodies in the nation. Grace Hospice has served hundreds of patients and families in the St. Clair County area.
Grace Hospice Services
Our care includes a team of doctors, nurses, social workers, counselors, hospice aides, chaplains, and trained volunteers. They work together focusing on the patient’s needs, whether physical, psychological, or spiritual.
- Physician services for the medical direction of the patient’s care
- Regular home visits by registered nurses and licensed vocational nurses, hospice aides, and homemakers
- Nurse available 24/7 for any questions or needs that arise
- Social work and chaplain visits to assist with final arrangements and bereavement care afterward
- Medical equipment such as hospital beds and oxygen
- Medical supplies such as bandages and catheters
- Medications delivered to the home for symptom control and pain relief
- Volunteer support to assist patients and loved ones
- Physical therapy, speech therapy, occupational therapy, and dietary counseling as needed

Lisa Hartman has been a nurse in the Blue Water Area for 23 years –
Her career started at Port Huron Hospital – working on the 5th floor in Med-Surg and Oncology followed by Hospice case-management.
She started in nursing management in 2008 in skilled nursing facilities and in Hospice. She currently is the Branch Director of Grace Hospice in Marysville.
The Grace Hospice Team is like no other – their care, compassion, and dedication to our patients amazes her every day, and she is so proud and blessed to work with each of them every day.
On a personal note, she has 2 daughters, Katie and Kristen and keeps busy with their sporting events including softball, basketball, and attending Equestrian meets with their horse “Ivy.”
